TERMS AND CONDITIONS
1. REGISTRATION
1.1 The website of “Astghik” Medical Center allows you to register a visit to the doctor of your choice, get medical advice online via message or video call, have a personal “Cabinet”, make use of the SOS button and other services provided by the website.
1.2 Registration at the company’s website is made online, with the use of a mandatory user-patient’s passport / identification card number, but the final verification of registration is made upon your arrival at “Astghik” Medical Center, during which you must produce your identity document, under which you made the registration.
1.3 The user-patient’s passport / identification card number is his / her identification code at “Astghik” Medical Center.
1.4 Until the final verification of the registration in the Medical Center, the user-patient can make use of all the services of the site. However, he/she cannot get their answers, except for the doctor’s online consultation service (chat, videocall).
2. USE OF INFORMATION PROVIDED BY YOU
2.1 Be informed that the personal data received from you are used exclusively for the provision of the services of the Center, ensuring the required security, personal identification and for the implementation of all the goals related to the activities of the service.
2.2 The e-mail address provided by you and the phone number can be used to send the news, promotions and offers of the Center, which you can unsubscribe, and you will automatically receive notifications of the application to your mobile phone.
2.3 Occasionally we may request information through enquiries or competitions. Participation in such enquiries or competitions is voluntary. You may or may not provide the required information at will.
3. USER-PATIENT’S RIGHTS
3.1 The user-patient (hereinafter referred to as the Data subject) is entitled to receive information about his / her personal data, data processing, processing grounds and purposes, data processor, his / her location, as well as the circle of persons to whom personal data may be transferred.
3.2 The data subject has the right to access his / her personal data, require from the data processor to correct, block or destroy his / her personal data, if the personal data are incomplete or inaccurate or outdated or obtained illegally or are not required for processing purposes.
3.3. The data processor, who in this case is “Astghik” Medical Center, must provide the data subject with personal information in an accessible form and must not contain personal information about another data subject.
3.4 Personal data is provided to the data subject on the basis of a written request from the data subject or his / her representative acting by virtue of a Power of Attorney or legal representative. The request can be submitted electronically with a verified digital signature or by arriving at “Astghik” Medical Center.
3.5 Information is provided to the data subject on a free of charge basis.
3.6 The data and documents on the personal health of the patient are kept in “Astghik” Medical Center until the death of the patient and a year upon death.
4. PERSONAL “CABINET”
4.1 There is a personal “Cabinet” on the site, which is designed for providing patients with information, data, documents about their personal health.
4.2 The personal “Cabinet” is free, but the services included in it are subject to payment in the appropriate amount.
4.3 In the personal “Cabinet” section the user-patient can not only get acquainted with his / her documents, but also order services provided by “Astghik” Medical Center, pre-register a visit to a doctor, as well as receive online consultation via chat and via video call.
5. CONTACT WITH THE DOCTOR
5.1 There are two ways to communicate with a doctor on the site: via chat and videocall, which is available to both registered users of the site and site visitors.
5.2 These services are currently free, but subject to change.
5.3 In the chat section of the website, the patient can contact the doctor and get free medical advice.
5.4 The messages are not publicly available, they are open only to the patient and doctor to whom he / she has applied / selected for an online consultation.
5.5 Medical advice is provided via videocall with the direct participation of the doctor and the person receiving the consultation.
5.6 The recordings are not publicly available, but are stored in the patient’s personal "Cabinet," which the patient can view at any time, immediately after the consultation.
6. EMERGENCY CALL (SOS)
6.1 SOS alarm service is available on the site, which is intended for emergency call registration and making use of the service of “Astghik” Medical Center emergency response team.
6.2 Upon registration of SOS calls, the medical staff of “Astghik” Medical Center will arrive at the place specified by the call registrar within a reasonable short period of time.
6.3 SOS service is paid, you can order it for a monthly, quarterly or annual fee.
6.4 The monthly fee is X amount and the annual fee is Y amount. You can pay for it via a bank card assigned to your page, or simply by arriving at a Medical Center and making a payment.
6.5 Taking into account the fact that after registering the SOS call, the doctors, police and emergency response teams of the Ministry of Emergency Situations will arrive at the place you mentioned, be informed that in case of a false alarm, the call registrar shall be held liable in the form of penalty. The penalty is X amount, which will be charged from the bank card assigned. In case of failure to assign a bank card, the amount equal to the penalty will be charged by you for ordering any service and paying for it.
7. PAYMENT SYSTEM
7.1 You can pay for the services provided by “Astghik” Medical Center both online and by visiting the Medical Center.
7.2 Online payment is made via a bank card.
7.3 You can attach your bank card details both after each payment and once by clicking the “Save” button.
7.4 The company, by reducing all the risks and observing the safety rules, excludes the automatic collection of money. In view of the following, you consent to the payment being confirmed each time a payment confirmation code consisting of a four-digit number is received in the form of a message to the telephone number you indicated. Only after receiving the code and confirming it in the appropriate payment field will the service fee provided by the Medical Center be charged from your bank card.
7.5 Be informed that the cases of medical care for which the citizens submitted the referrals to the medical organization after choosing the paid method of treatment, concluding the contract for paid services or making the relevant payments are not subject to compensation within the framework of free medical care and service guaranteed by the state.
7.6 Thus, you agree that your payment will not be refunded after you choose the paid treatment method, conclude the paid services contract or make the appropriate payments, even when you have received a state-guaranteed referral for free medical care.
8. DISPUTE RESOLUTION PROCEDURE
8․1 In case any disputes arise between the parties, they shall be settled through negotiations directly between the Patient and “Astghik” Medical Center or through representatives.
8.2 In case no consensus is reached through negotiations, the disputes shall be referred to the Court of General Jurisdiction of the Republic of Armenia for settlement thereof.
9. FINAL PROVISIONS
9.1 These Rules and Terms may be changed unilaterally at any time without prior notice. You will be notified of changes by email if you subscribe to the news.
9.2 These Rules and Terms, as well as all its amendments, shall be approved by the General Manager of the Company and shall enter into force after publication thereof on the Company’s website at http://www.mcastghik.com.
9.3 From the entry into force of these Rules and Terms, the previous editions shall be considered invalid.
